Self-Pay

If your insurance plan is not included above, self-pay services can be arranged.

I am also able to provide any necessary paperwork, such as a superbill, for you to seek reimbursement from your insurance directly. In some cases, I can also submit claims to your insurance as a courtesy. While insurance may include out of network coverage, coverage depends on your deductible and there is no guarantee they will reimburse the costs.
